Levorphanol is not a recognized metabolite of any other drug. Levorphanol is an enantiomer or optical isomer of dextrorphan, a metabolite of dextromethorphan. An enantiomer exists when two chemical structures share a common chemical formula and molecular mass but are structurally mirror images (left-handedness versus right-handedness). LC-MS/MS confirmation may identify dextrorphan as levorphanol or the obverse. Because of their chemical similarities, levorphanol presence should be evaluated carefully along with patient history of dextromethorphan use. A levorphanol standard of this molecular mass can be used so any of this mass detected will be identified as levorphanol.
